Insurance

We will be happy to file with your insurance company if you have provided us with the necessary information at the time of service. We need you to fill out our patient registration form, then we can submit the essential information. We also need a copy of your current insurance card to reduce any inaccuracies with filing.

Please keep us informed if there are any changes in your insurance. It is also important for you to know the benefits of coverage for your insurance, including co-pays, and deductibles.

Payment — If you have not yet met your deductible, or if you have a co-payment with your insurance, we require payment in full. If you do not have insurance or if your insurance does not cover certain services, payment is required at the time of service.

If payment at the time of service presents a financial burden, a payment plan may be arranged with our patient account coordinator.

Any balance remaining at 60 days needs to be paid in full unless other arrangements are made with our office.
